We recently migrated our patient management portal to a new hosting platform and are experiencing technical difficulties. Please know that your data remains secure and we have retained all patient therapy records. While we work through this, we ask that you utilize our patient Prescription Form to reorder current or start therapy for a new patient. For new patients, we need both the Prescription Form and the Test Results Form. For existing patients, we only need the Prescription Form. In the event you do not have the Form, download it from here.
You may fax any order to the number on the form or scan and send it to orders@oasisallergy.com.
